Tattoo Procedure and Pain Management: I understand that receiving a tattoo involves the use of needles, which may cause discomfort or pain. I consent to the use of a topical anesthetic or other pain management techniques if available, understanding that this may not eliminate all discomfort.
Age Requirement: I certify that I am 18 years of age or older. For clients under the age of 18, a legal guardian's consent is required.
Health Conditions: I understand that receiving a tattoo may involve risks and that if I have any existing health conditions (e.g., allergies, heart conditions, diabetes), I should consult my healthcare provider before proceeding with the tattoo. Rated Art Tattoo Studio is not responsible for any adverse reactions or complications resulting from receiving a tattoo.
Aftercare Instructions: I acknowledge that Rated Art Tattoo Studio will provide me with aftercare instructions for proper healing and preservation of the tattoo. I understand that failure to follow these instructions may result in complications, and Rated Art Tattoo Studio will not be held responsible.
Tattoo Design and Placement: I understand that Rated Art Tattoo Studio will provide a general representation of the tattoo design, and that slight variations may occur as part of the artistic process. I am satisfied with the size, placement, and design of the tattoo as discussed with the artist.
Artist's Right to Refuse or Modify Tattoo: I understand that the tattoo artist has the right to refuse or modify the tattoo design if it is deemed inappropriate, offensive, or against their professional judgment.
Consent and Release of Liability: I hereby release, discharge, and hold harmless Rated Art Tattoo Studio and its artists from any liability, claims, demands, or actions that may arise from or are connected in any way to the tattoo procedure, including but not limited to accidents, errors, infections, allergies, or any other complications.
Sterilization and Safety Measures: I understand that Rated Art Tattoo Studio implements strict sterilization and safety measures to prevent the transmission of bloodborne pathogens. All equipment used is disposable or properly sterilized using industry-standard methods.
